It was originally supposed to air a week later on Oct. 16, but the network decided to move up the premiere date. However, it's unclear why ABC decided to make this decision. We're not complaining, though, because that just means we get to see the new season even sooner!
The twenty-second installment is set to air after 911 season 9 and 911: Nashville. 911 season 9 will kick off the night at 8/7c, followed by the premiere of 911: Nashville at 9/8c and then Grey's Anatomy season 22 at 10/9c. If you miss the premiere, you can stream the episode the following day on Hulu. The same goes for future episodes as well.

Who could be in the Grey's Anatomy season 22 cast?
So, here's the thing about the Grey's Anatomy season 22 cast. An official cast list has not been revealed. The only confirmed cast members are Ellen Pompeo (Meredith Grey), Caterina Scorsone (Amelia Shepherd), Anthony Hill (Winston Ndugu), and Alexis Floyd (Simone Griffith). All of these characters appear in the new Grey's Anatomy season 22 production stills that ABC released in early September. Trevor Jackson, who joined the medical drama last season, is also confirmed to return, as evidenced by his appearance in one of the new photos.
Camilla Luddington, who portrays Jo Wilson, is basically confirmed to return as well after she spoke about the filming process for season 22 in an interview with E! News back in August 2025. The rest of the main cast is also expected to come back given they've signed contracts extending into season 22.

Grey's Anatomy showrunner Meg Marinis shared in an interview with TVLine that some characters from previous seasons will also make an appearance in the twenty-second installment. Although she did not give any names, we've already started speculating who they can be. We hope one of those characters is Mika, who was played by Midori Francis. Mika left Grey Sloan in the first half of season 21. She and Jules still have some unfinished business, and we'd like to see how things play out.
How did Grey's Anatomy season 21 end?
While different doctors were around the hospital tending to their patients, the season 21 finale mainly focused on a desperate mother who, fearing for her daughter’s survival, resorted to extreme measures to force the doctors to continue operating on her. Her actions escalated into a tense hostage-like situation, with Amelia, Simone, and other staff caught in the middle.
Although the girl pulls through after a successful operation, chaos erupts when the highly flammable acetylene tank the mother used to threaten the doctors topples over, triggering an explosion that engulfs the entire OR floor in flames. Those on the OR floor at the time were Link, Monica, Miranda, and Jules. While they're all the most at risk, Marinis shared in a recent interview that anyone in the hospital at the time of the explosion is in danger.
Here are more important things that happen in the season 21 finale:
- Owen and Teddy’s relationship hits a breaking point. Witnessing Owen’s deep bond with Nora, Teddy realizes she can’t continue their marriage and chooses to prioritize herself rather than remain in a relationship where she doesn't feel appreciated.
- Simone and Luca reconcile, but Simone is then placed in a weird situation when the guy she hooked up with behind Lucas's back turns out to be a new intern at the hospital.
- Jo reveals to Link that they're having twin girls.
- Meredith strikes a deal with Catherine that allows her to spend her summers in Seattle working at Grey Sloan, while also still being able to pursue her Alzheimer's research.
