If you expected Doc season 2 to open right where season 1 left off, you’re in for a major surprise. Season 2 kicks off with an intense action sequence as we see police assembling and learn that there has been a shooting in the hospital that has injured one member of the hospital’s team. Before we find out who has been shot, we rewind the clock by six hours.

As we start the day, Amy is trying to get in touch with both Jake and Gina, who are both giving the silent treatment. When she gets to the hospital, she finally catches Jake, and she tries to explain away the kiss he saw in the finale between her and Michael. While Amy suggests that it was nothing but a goodbye kiss that came up in the moment, Jake isn’t having it and walks away after telling her he misses the woman he fell in love with.

Amy’s day begins normally as she goes to see a patient by the name of Rosie, who is in need of a heart. It seems Amy has a history with the patient’s dad, Alex, but has no memory thanks to her accident. We don’t quite know the backstory, but it’s clear he is not a fan of Amy. As she and TJ perform some routine scans, they find a spot on her lung that will require further scans and could result in Rosie coming off the transplant list.

Upon discovering the spot on her lung, Amy and TJ inform Michael and he agrees with TJ that they have to notify UNOS.

We flash back to the past, and find out that Amy told Alex not to pursue the transplant years ago when she was treating Rosie. She assured him that his daughter might not survive the transplant and suggests he enjoy the time he has with his daughter rather than rolling the dice.

TJ is shot when Rosie’s transplant is threatened

As we return to the present day, Amy and TJ inform Alex that while they’re waiting to find out what the spot was, they had to report it to UNOS. He lashes out at Amy as Jake watches from afar, and he assures Rosie he won’t let it happen again, not this time. We see them wandering through the halls, honing in on a security guard. He grabs the guard’s gun, takes his radio, and pushes him into the elevator as he then trashes the radio.

Jake sees the exchange and confronts Alex, which is when he hits him in the head with the gun. TJ rushes him and begins fighting for the gun. During the exchange, TJ is shot and it’s not looking good. After Amy convinces Alex to allow someone to get TJ help, he orders Jake to take TJ away to get treatment before calling Michael. He demands the heart be given back to his daughter, and threatens that others will get hurt if they don’t ensure it. While Michael is able to convince Alex to clear the floor, he keeps nurse Julie, Sonya, and Amy to retain some leverage.

Amy gets the chance to confront Alex about their connection in hopes of getting information on their past, and he informs her that their conversation all those years back scared them so much, they went against Rosie’s cardiologist and turned down the transplant.

Michael lies to get UNOS to agree to the transplant

UNOS agrees to send the heart after Michael withholds some information from them, so Michael sends Jake to retrieve the heart. We learn that Michael had to lie to get UNOS to sign off and has chosen not to call the board to get them involved. He’s juggling this conflict with his wife going into labor early, and it’s clear his actions in the premiere are likely going to come back to haunt him.

After performing a successful test with Sonya’s help, Amy convinces Alex to allow her to call the OR to check on TJ. He agrees, but it’s not good news as he’s losing circulation and they don’t know if he’s going to make it.

When Jake arrives at East Side Hospital to pick up the heart, the nurse at the front desk isn’t certain about things given the news coverage about the hostage situation at the hospital. Jake assures her that the transplant is happening on a different floor, but she calls Michael to confirm. He does, but we hear her saying she’ll need it in writing – again foreshadowing Michael’s actions in lying to UNOS will come back to bite him.

Flashing back to the past, we go back three years and see Amy visiting Alex. She learns that his wife has passed away from COVID and admits to him that the advice she gave him was the advice she’d want as a mother. He’s not having it and says the cardiologist gave his advice as a doctor, something Amy should have done too. We return to the present day as Amy tries to convince Alex to let Julie and Sonya go. Her pleas get through as he allows Julie to leave.

Does TJ die in the Doc season 2 premiere?

At East Side, Jake learns that the family of the transplant patient is having second thoughts. Knowing what’s on the line, he gives them an emotional speech, and it works as it was the gentle push they needed. Jake arrives with the heart, but Alex is spiraling and pulls the gun on Amy. When they get word to Alex that the heart has arrived, he demands that it be sent up by someone he knows, and Jake volunteers.

Before he heads up, Gina tells him she just got off the phone with Rosie’s former cardiologist and he needs to know the story.

When Jake arrives with the heart, he informs Alex that they can’t perform the transplant. This is when Amy comes rushing out and reveals that Rosie has abnormal vessels in her lung and the transplant wouldn’t work. It turns out Amy withheld the information years ago because she didn’t want to take away his hope and wanted him to have those years with Rosie he might not have otherwise gotten if they did the transplant. Rosie’s dying, and Amy confronts him about how he really wants to spend his last moments with her.

Jake reveals that when Amy first treated Rosie, she had just lost her kid and was trying to save Alex some of the pain she was going through, as he knows her and her heart. Amy says they can give Rosie something to revive her for a few minutes so Alex can say goodbye. He sends Jake away with the heart so it can go to someone who needs it, and gets the chance to say a final goodbye before the cops take him away.

As the episode ends, we learn that TJ has pulled through his surgery, but it’s not clear whether he’s 100% out of the woods. We’re just relieved he’s alive and wasn’t killed off!

In the final moments, we also learn that Amy had a brief memory of a conversation she had with Alex years ago during the intense ordeal. Amy wants to try going into the tank in hopes of regaining more memories from her lost years, and manages to convince Gina to allow her to as the episode ends. It’s a moment that gives us hope that Amy will be able to regain some memories in the season ahead, but we’re curious to see how those memories might impact her life and just what memories will come flooding back.

Wherever the journey takes us, we're so excited to see what season 2 holds for Amy! If season 1 put Doc on the map, season 2 proves its only getting started, with the second season shaping up to be bigger, bolder, and even more unforgettable. Another can't miss season is officially underway and fans won't want to miss new episodes Tuesdays at 9/8c on FOX!
